This is a customer satisfaction survey, meaning it's goal is to gain knowledge and insight into how customers view their experience during their recent visit to Smoothie King.

Rewards or "prizes" are often given for completing customer surveys such as this (check your receipt for details). This is what I like to call a "Survey Reward".

"Survey Rewards" are given to you as an incentive to give your valuable feedback as a customer.

If your receipt states that there is a reward (such as a free coupon, or sweepstakes entry) getting your free Survey Reward is simple.

To get your discount, have a recent Smoothie King receipt handy, and visit, enter the store number, date, and time from your receipt, and then click “Start” to tell them what you think. Upon completion you will receive a validation code, jot that down on your receipt.

    Had to throw them away. Had some type of gritty almost sand like substance
    that came up through the straw.
    Have never had this before.

  4. It would really be helpful if you all had a CLEAR list of all the smoothies that are actually $5 on Friday’s. Every time I go on a Friday for the $5 32oz. smoothie it’s not as advertise. stop confusing your customers. just make a list!

  5. Smoothy King 0269, i requested the lowest calorie 32 oz Gladiator and nobody knew what to recommend . They said it up to the fruit but did not know what to recommend either. Nobody had any clue about calories at all.

  8. The Smoothie King that I visit frequently is #1364 and I love the smoothies and the employees that work there. Today I was served by Kelsey and Kelly and I enjoy every second that I am being served there . They not only provide extraordinary service (they always remember my favorites), they are very intelligent and provide very interesting conversation. I am always amazed at how they can manage several customers at the same time. I learned today that Kelsey will graduate in December and may move on. If she does, she will be greatly missed. It is always a memorable experience when I visit your store.

  9. The customer service was horrible. I went to the smoothie king #1625 I got to the drive through at 8:48 pm the young lady who took my order clearly did not wan to be there. I finally got to the window and checked out at 8:59. The whole time they had the window open while they cursed and complained about not wanting to leave late. They were very unprofessional.

  10. I take my granddaughter twice every month to Smoothie King in San Antonio Texas. I live
    out in the country on the outskirt of this big city. She receives SSI monthly and this is her monthly routine. She is 26 yrs old, she loves your smoothie. (I like it too but too expensive). My granddaughter will even buy an extra smoothie to bring back home for the long week so you can say she has one smoothie every week Lol. (that saves me gas)

  11. I love the Smoothie King I patronize in my neighborhood. It’s a bit hard to navigate due to all the traffic and the lot it is located, but all things considered it is worth the extra effort. I am a self described health food “nut”. I try to eat and drink as healthy as possible. This is not an easy thing to do based upon the all the local “fast food” options which seem to be popping up every week. The staff are always cheerful and efficient. Also, patient, as the options are mesmerizing. I attempt to try a different smoothie each time and it does take me a little while to decide as they all sound scrumptious. I feel so energized after I consume a smoothie, they could be a meal replacement. I wish they could expand and offer more locations. Thank You Smoothie King!

    Every Friday 3-5 of us will visit the Smoothie King in Roswell, Ga off Old Alabama Rd.
    We have been going there Since March up until 3 weeks ago. The service is ok. There’s a few young ladies who really don’t act like they like there job. They are all always on their cell phones. One time I ordered over the phone and she was talking to her co-worker and not listening to my order. The counters are never clean. There is always straw wrappers on the floor. The straw that broke he camels back was when I saw a ton of fruit flies by the blenders. I had ordered 4 drinks they called my name but only had 3 ready so I stood there for a minute and noticed the fruit flies. I went back to work and told the girls we would not be ordering there again!!! They weren’t really happy but they also didn’t want bugs in their smoothies.

  13. The destrehan smoothie king is great. Jenesis is a great worker. She is always smiling and friendly. She has great customer service. I also told her manager Mr. Grims she’s a keeper. Smoothies are always great.

    Used to go to Smoothie King Fells Point almost everyday. Not anymore. High changeover in staff. I do not feel welcome Just interested in your money. Elderly disadvantaged homeless are frowned on at this location. I can only hope they are blessed with more customers that they do like.

  17. I went to Smoothie King in Cinnaminson, NJ, and requested the Blueberry Heaven on Feb. 6, 2018, @ 5:56 pm, without the syrup. All other ingredients were to be included. I wanted the smoothie on my way to work this morning. I tasted the smoothie and it was disgusting. This has never happened before, you could not even taste the blueberries. I took it back on Feb 7, 2018, and the manager would not replace the smoothie because I did not bring it back last night. I explained why he still would not take it back. I paid $7.68 for nothing, as he had no problem taking the smoothie back to throw away, but would not return the money I paid for it, as he stated he couldn’t return my money. I will never go to Smoothie King again.
